71-Applebee's Neighborhood Grill & Bar No. 211, 168 Paul Huff Parkway, 8/1/18-"7: Observed waitress handle RTE cheese sticks with bare hands to re-position them on plate. I was unable to reach the employee in time to stop them from taking them to the dining area. Discussed and educated," "8: No paper towels at hand sink at front of kitchen," "14: No chlorine sanitizer reading from auto dishwasher. 3CS will be used," and "20: Diced tomatoes, pico in prep cooler 45-47 degrees. Prep cooler thermostat will be adjusted down."

81-Captain D's, 3030 APD 40, 8/22/18-"6: Employee handled raw fish, removed gloves, did not wash hands, put new gloves on and handled RTE fish." "Employee handled raw fish, did not change gloves and wash hands, handled tray to hold RTE fried crumbs. The employee then handled the fully cooked fish." "Employee handled raw chicken then handled container and spigot of water cooler that is used to take water for prep with rice an various batters," and "Rice and green beans 130. Hot holding unit had accidentally been set to low. PIC EMB 3 lbs."

79-Days Inn-Breakfast, 2550 Georgetown Road, 8/23/18-"1: PIC [person in charge] did not know cold holding requirement," "2: No health policy available," "8: Hand sink inaccessible due to bags of bread set in it," "20: Sausage biscuits set in tray with no temperature control at 6 a.m. 60 degrees. EMB 1 lb. Discussed using TPHC in detail. PIC states they will start using time," and "20: Hard boiled eggs 50. Set on ice at 6 a.m., but ice has melted since. Will rapidly cool in working cooler."

94-Domino's Pizza, 2507 N. Ocoee St., 8/30/18

78-Dos Bros, 4450 Frontage Road, 8/24/18-"11: Cans of roasted red peppers very damaged at seals. EMB 11 lbs.," "14: Metal containers only dipped in Quat sanitizer for a few seconds. COS Placed back in sanitizer for at least 30 seconds," "14: Visibly dirty containers stored with clean containers on racks. COS will be re-cleaned and sanitized," and "21: Medium salsa prepared with fresh cooked tomatoes and peppers improperly date-marked. COS."

92-Douglas Inn-Breakfast, 2600 Westside Drive, 8/31/18-"8: No paper towels at handsink."

72-Hardees, 2020 Spring Place Road, 8/14/18-"1: PIC not ANSI certified. PIC did not know required cook temp for chicken; answered 140-160. PIC was unaware of specific employee health policy," "2: PIC is not aware of employee health policy, she states they likely have one, but she is unable to locate it or describe it," "4: Confirmed employee drink in unapproved container stored over bread and freezer. COS," "13: Live roach observed by buns and over freezer. Several more live roaches observed at biscuit prep table. Establishment last treated almost a month ago," and "14: Observed food container being put in Quat sanitizer for only 10 seconds instead of the minimum of 30 seconds. COS items placed back in sanitizer for required contact time."

86-Hardees, 2020 Spring Place Road, 8/17/18-"6: Observed employee washing only a portion of their hands for less than 10 seconds. The employee then turned the faucet off without a paper towel and the faucet handles are very dirty with visible build-up. Observed multiple times. COS: educated on proper handwashing technique," and "20: TCS creamer 55l EMB 1 lb."

78-Lee University Cafeteria, 1120 N. Ocoee St., 8/30/18-"4: Employee drink (open can of soda) stored by pepper and over other vegetables in reach in cooler. PIC COS," "8: Observed water from hand sink splashed on to clean equipment. Equipment will be cleaned and splashguard will be added," "8: Paper towels needed in employee restroom and soap needed in one public restroom. COS," "20: Yogurt 49. PIC EMB .5 lb. TPHC will be used going forward. Procedure written," "20: Lettuce, whipped butter 45. Will be moved to another cooler to rapidly cool. COS Cooler will not be used for TCS foods," and "26: Unlabelled chemical spray bottle. COS."

89-Leisure Time Bowling, 2739 Keith St., 8/6/18-"Unlabelled chemical spray bottle. COS."

92-Ms. Nancy's Day Care & Pre-School Kitchen, 197 Neely Circle, 8/13/18-"14: Unapproved sanitizer wipes used to wipe down food prep table, etc. Educated on proper sanitizer."

86-Nela's Tacos/La Michocana, 247 Keith St., 8/3/18-"19: Pork 119, corn 130; was set on floor in office several minutes ago. Corn will be reheated to 165. Pork PIC EMB 1 lbs.," "21: Lengua cooked 2 days ago not date marked. COS," "21: RTE spinach opened two days ago not date marked. COS," and "26: Unlabelled chemical spray bottle. COS."

86-Quality Inn Breakfast, 153 James Asbury Drive, 8/27/18-"20: Bag of scrambled egg set on counter at unknown time this morning at 65 degrees. EMB 2 lbs.," "20: Cream cheese, yogurt, butter placed in small refrigerator in dining area 4-5 hrs ago at 54 degrees. EMB 4 lbs.," and "26: Air sanitizer spray stored on shelf with containers of coffee and butter spray over microwave."
