The Cleveland schools system has announced a date for kindergarten registration for the 2012-2013 school year.

Registration is scheduled from 1 until 3 p.m. Friday, May 4. Children who will be five on or before Sept. 30 should register in their respective zoned school.

For registration, parents need to bring their child’s birth certificate (or record of birth from the hospital), the child’s Social Security card; record of a recent physical (since Jan. 1, 2012; and proof of residency (rent receipt, utility bill or some proof of residency.

Before entering school in August, every child should have an official birth certificate, a recent medical examination, and all immunizations recorded on the Tennessee Permanent Immunization Record.

These immunizations include two MMRs, three hepatitis B, five DPT, four polio, two hepatitis A (effective July 2011) and two chicken pox vaccinations.

If the child has had chicken pox, parents will need a letter verified by his/her doctor along with the dates the child was infected.

School officials say it is important that parents register their child for kindergarten on May 4 in order for the child to have a place in their local zoned school.

They also say it is not necessary to bring the child to the registration. Parents who may have questions should call the child’s elementary school.
